Athletic Physicals


A Medical History and Pre-Participation Examination Physical is required for ALL student athletes, grades 7th-12th, for the 2021-2022 school year.

  • Medical History and Physical forms should be completed on the DISD pink form.

  • Free physicals will be offered for DISD student athletes at the Denison High School Field House/ Indoor Facility on Wednesday May 5, 2021.  High School students will start at 4:15 pm and Middle School students will start at 4:45 pm. 


Athletic Paperwork:

ALL forms EXCEPT for the Pre-Participation Physical and Medical History will be completed and submitted using the RankOne online based software system. 

  • To access RankOne, go to:  htts:// and proceed to the online forms.

  • Please ensure the 2021-2022 school year is selected.     

  • Do not use athlete nicknames on forms.  Use the student’s full legal name.

  • The athlete’s student ID number is required on each form.

  • There are 7 forms total that must be completed.

All forms (Online, Medical History, & Pre-Participation Physical) MUST be complete and on file PRIOR TO PARTICIPATION in any practice, scrimmage or contest before, during, or after school.


Brent Whitson




Denison ISD is proud to welcome new Athletic Director and Head Football Coach Brent Whitson to the Yellow Jacket family. Whitson brings a tradition of athletic excellence and strong work ethic to the Denison program. In rebuilding programs and winning championships across Texas and Oklahoma, the Whitsons have come to recognize that they are “community” people and look forward to the opportunity to be involved in Denison, Texas as members and leaders of this growing city and school district. While Whitson desires an increased presence in the community, he understands the core responsibility of the position.

“Establishing and maintaining relationships with players and coaches is the most rewarding part of our business. Teaching and guiding players and students while developing and mentoring outstanding coaches in all our programs is exciting every day,” Whitson says.

Whitson plans to direct the Denison ISD athletic program based on three key beliefs…

  • BELIEVE IN WHAT WE DO Because what we do works.  It is well planned and trusted.

  • BELIEVE IN WHO WE SEND TO DO IT Because that is my teammate when it is not me.

  • EXPECT TO WIN Because I believe in what we do…

Whitson says players recite and live this program mantra.  It is chanted, shouted, and spoken in hushed tones by students who have been taught what this level of belief can accomplish.

Whitson brings a rich tradition of athletic success to the Yellow Jacket and Lady Jacket program. He comes to Denison ISD from South Grand Prairie High School where he served as Head Football Coach and Campus Athletic Coordinator since 2011. The Warriors made the playoffs in 2014, 2015, 2016, 2017, 2018, 2019, 2020. Whitson was also the Richardson High School Head Coach and Campus Athletic Coordinator from 2007 to 2011, making the playoffs in 2009 and 2010. Prior to coming back to Texas, Whitson served at Shawnee High School in Shawnee, Oklahoma as Head Coach and Assistant Athletic Director from 2000 to 2003 making the playoffs in 2000 and 2002 and earned a coveted State Championship in 2003. Before moving to Shawnee, Whitson was at Broken Arrow High School in Broken Arrow, Oklahoma as the Head Coach and Assistant Athletic Director from 2004 to 2006 where the Tigers made the playoffs in 2004, 2005 and 2006 and were a State Semi-Finalist team in 2005. Whitson began his coaching career at Weatherford High School in Weatherford, Texas as Defensive Coordinator from 1998 to 1999 and Assistant Head Coach from 1996 to 1999. The Weatherford Kangaroos made the playoffs 1996, 1998 and 1999 and were Regional Finalists.

Coach Whitson earned his Bachelor of Science degree in Business from East Central University in Ada, Oklahoma in 1984 where he graduated with honors. He is a member of the Texas High School Coaches Association, a member of the American Football Coaches Association and a member of the North Texas Football Coaches Association. In 2003, Whitson was named the Oklahoma Football Coach of the Year by The Daily Oklahoman, The Tulsa World, and the Oklahoma Coaches Association.
